CERTAIN Program Series

CERTAIN (Checklist for Early Recognition and Treatment of Acute Illness and iNjury) is a quality improvement program that offers a systematic approach to the initial assessment and evidence-based management of the critically ill or injured patient. A recent international (34 ICUs from 15 countries) quality improvement trial showed how CERTAIN implementation is feasible and associated with better adherence to best practices of critical care processes, shorter ICU and hospital lengths of stay, decrease in complications, and lower mortality. Built on strong preliminary data and state-of-the art implementation science, the CERTAIN programs have the potential to transform ICU practice globally.

CERTAIN Simulation Workshop

Experience the systematic, evidence-based CERTAIN approach to the initial resuscitation and ongoing care of acutely and critically ill patients using a model that is simple to implement within an ICU environment. Our simulation workshop provides participants with the opportunity to experience and reflect on our recommended approach and to discuss the best ways to incorporate these practices in different ICUs. Additional opportunities during this workshop include interactive discussions on best approaches to shared decision making, humanizing the ICU, and challenging controversies commonly faced in critical care practice. Additional module options include rapid use of point of care ultrasound to guide resuscitation, mechanical ventilation, and targeted shock management. The remote simulation has been shown to improve both individual skills and teamwork.

CERTAIN Coaching

CERTAIN Coaching is a year-long ICU remote coaching program offering a longitudinal, weekly, and interactive virtual learning experience with Mayo Clinic critical care experts and CERTAIN faculty. This program offers a monthly core critical care curriculum, weekly case-based discussions, journal clubs, along with opportunities to develop, discuss, and collaborate on research and quality improvement projects.

By using state of art asynchronous online learning, remote simulation training, competency assessment, and coaching, this novel tele-education program is designed to maximize education efficiency and effectiveness, and ultimately improve meaningful patient outcomes. Implementation of CERTAIN coaching in critical care setting with limited resources resulted in lasting changes in structure and processes and was associated with improvement in outcomes of critically ill patients and marked cost-savings.

Weekly Video-Assisted Case-Based Presentations: Weekly hour-long case-based discussions will be conducted using Zoom web conferencing platform. Expert moderators proficient in local language will facilitate interdisciplinary and inter-institutional discussion of each case.

Outcome evaluation:  We evaluate the program effectiveness based on Kirkpatrick model 1) Reaction: learner participation and engagement; 2) Learning: educational outcomes (knowledge and skill); 3) Behavior: bedside application of evidence-based care processes in learner institution. 4) Results: patient-centered outcomes (length of stay, mortality). The participating institution has the option to conduct research project to study the impact the education program.

CERTAIN Faculty Development (Train-the-trainer)

CERTAIN Faculty Development course is designed for those willing to advance their skills in teaching others to implement CERTAIN program in various hospitals. This requires advanced skills to conduct analysis for training needs, skill gaps and goals of organization, and to design customized curriculum and evaluation metrics to improve clinical skill.  We will cover topics listed below:

  • Fundamental of medical education

  • Incorporate system thinking and design thinking for program development

  • Change management principles

  • Practical project management skills

  • Engagement of social media for medical professionals

  • Data analysis principles

Completion of the Faculty development course and teaching two CERTAIN courses with CERTAIN Faculty is required for any candidates to become CERTAIN Faculty. The CERTAIN Faculty provides committed individuals with opportunities to educate and lead the course in his/her own hospital and countries.   

 

CERTAIN International

One or two days live education program with didactic lectures, simulations, workshops. Participants will learn from the practical experience of an international group of faculty experts, who have implemented this program in the hospitals across five continents. This program will be delivered by Mayo and regional faculty on selected US and international locations in partnership with host institution or organization. The course have been conducted in India, Croatia, Slovenia, China and Vietnam.

CERTAIN Rochester

Implementation of best acute care practices to drive safety and quality improvements remains challenging in many healthcare systems.  The CERTAIN program offers healthcare teams a validated, bedside decision support tool to standardize best approaches for the evaluation and treatment of acutely ill patients.

This 1.5 day course provides participants an interactive opportunity to learn the CERTAIN approach, including hands on simulation based training. Participants will also learn from the practical experience of an international group of faculty experts, who have implemented this program in intensive care units (ICUs) across five continents over the past 10 years. Hospital tours is also included the workshop.
